In our ‘Next of Kin’ Care Model- you will move into the residential home of a nurse that will help you with bathing, food preparation, transportation, medication management, and emotional support. We want to know about your cultural identity and family traditions to place you in a home where you can have a sense of familiarity and comfort.

Can you send a Nurse to help me at my house?

No. Unfortunately, if you would like this type of care, you must move into a different home.
How much does this cost?

Monthly range is from $5,000-$12,000 depending on the level of daily care you need. If you can’t afford it, this service can be covered by Medicaid. Contact your insurance company or call your local Area Agency on Aging & Disability.
Do I have to move?

Yes. You will have to move into a new home where you will have a caregiver who understands and cares for you and new friends and family who will embrace your preferences for food, music and fun.
What does daily care look like for me?

What do YOU want help with? We will work with you come up with a plan that works for your preferences.
What type of nurses are available?

There is no limit to the variety of education, experience, and heart of the Nurses in our Network.
How long does it take for me to move-in?

The process can be as short as 3 days and some are more exploratory in nature and go on for years. The average time from inquiry to move in date is approximately 3 months.
